Driver Vehicle Inspection Reports (DVIR)Procedure
Driver Responsibilities:
- At the start of your trip, you must obtain the DVIR booklet from the power unit and document holder [trailer]. Part of the pre-trip inspection requires you to review the previous DVIR for each unit, then sign and date it [TOP copy] to acknowledge receipt and review of the previous DVIR.
- Where defects are recorded, you must satisfy yourself that all defects have been repaired, initialled and signed by the person who completed the repairs. Where no signature exists, you must contact either the on site shop supervisor [where applicable], or the local Branch Traffic Supervisor to confirm repairs were completed. At a minimum, all safety related repairs must be completed.
- The pre-trip inspection for each unit is then recorded on a new DVIR noting any equipment defects. Each inspection [pre-trip, en-route, post-trip] must be initialled and the time recorded. If repairs are required, you must contact either the on site shop supervisor [where applicable], or the local Branch Traffic Supervisor to have repairs completed.
- Upon completion of your trip, a post trip inspection is recorded on the DVIR for each unit, noting any equipment defects. Ensure the respective # for each defect is recorded on the right side of the report.
- The DVIR is a two (2) part document and the distribution of each is as follows:
- The TOP copy stays with the booklet and remains in the vehicle.
- The WHITE copy must be turned in with your bills. If there are no defects noted, the DVIR can be placed in the document tray. However, if there is an equipment defect, the local Branch Traffic Supervisor must be notified. Do not drop the DVIR into the document tray, since the equipment may need to be scheduled into the shop immediately. If the shop is open and the dispatch office is closed, take the WHITE copy to the on-site shop supervisor. They are responsible for contacting the local Branch Traffic Supervisor if an equipment scheduling conflict occurs.
- If both the shop and dispatch office are closed, and there are defects noted, contact the local on-call Branch Traffic Supervisor for instructions. Failure to notify them of an equipment problem could lead to a customer service failure
- When a wheel re-torque is required on the road, circle the appropriate codes for the wheel position [L=left-driver side, R=right, A#=axle number counting from the front, I=inside, O=outside, i.e. LA3O = left side, 3rd axle, outside wheel].
- In cases where a tire install has been completed in our shop, a separate DVIR will be prepared by the shop to record the wheel re-torque. This DVIR must also be signed and dated to acknowledge receipt and review of the report. This also serves as a reminder to have the wheel[s] re-torqued.
Customer Trailers:
- The Traffic Supervisor at the beginning of the load will issue a separate book.
- The completion of the DVIR is the same as above; however, the booklet must be returned to the dispatch office.
